The right person. The right support. The right fit.
Finding support should not feel like guesswork.
Connected Support learns what you need, identifies workers whose credentials, skills and availability fit, and stays involved once support begins.
Start a support requestYour journey with Connected Support
Each step is handled by our team alongside the platform.
- 1
Tell us what you need
A client, family member, guardian or authorized representative tells us what support is required. You can start online or talk to a person first.
- 2
We understand the need
We collect the type of support, the individual's needs, required skills and credentials, experience, schedule, location, transportation, preferences, communication needs and any relevant safety considerations.
- 3
We find appropriate matches
Our matching system reviews credentials, skills, experience, scope, availability, location, transportation, service type, client requirements and worker preferences. Price alone never determines a match.
- 4
Review your matches
You review worker profiles containing the information appropriate for you to see: name or approved display name, worker category, experience, skills, verified credentials, languages, availability and transportation. Sensitive documents are never shared.
- 5
Choose your match or matches
You can accept one worker or several — for example one worker for a weekday afternoon and another for a Saturday shift. More than one worker can support the same person.
- 6
Confirmation
When you accept a match we notify the worker and our coordinators, open a message thread, and add the shift to both your schedule and the worker's schedule.
- 7
Support begins
The worker checks in and out for each visit and completes visit documentation so there is a clear record of the support provided.
- 8
We stay connected
We keep managing scheduling, open shifts, notifications, documentation, credential status, matching, communication, changes in needs and replacement workers.
Needs change. Support should change with them.
Support is rarely static. We review what is working and adjust matching, scheduling and the support plan as needs change.
Getting started
We learn what you need and who needs support.
Finding the right fit
We identify appropriate workers and you review the matches.
Building routine
Support becomes consistent and familiar.
Reviewing what works
We monitor service delivery, documentation and feedback.
Adapting when needs change
The support plan and matching can be reassessed at any time.
The right fit matters
Support is personal. If a worker is not the right fit, tell us and we will review the situation and look for alternative qualified workers. We cannot promise that a replacement is always immediately available, but you will never be left without a plan.
- 1You tell us the current worker is not the right fit.
- 2Staff review the reason and handle the current arrangement respectfully.
- 3The matching system identifies alternative qualified workers.
- 4You review the new matches and accept one.
- 5The schedule is updated and everyone is notified.
We cannot promise that a replacement worker is always immediately available, but you will always be told where things stand.
